Your appointment has been canceled

I’m sorry you’ve needed to cancel your appointment. If you’ve had any problems or issues that have caused you to cancel, I’d love your feedback so I can improve my services in the future. If you have time to let me know the reason for your cancellation in the contact box below I’d greatly appreciate it.

If you’d like to reschedule your appointment:

Fill out the contact box below and we’ll set up a new appointment via email. Just list some convenient times in the message box and I’ll get back to you ASAP!

If you’re canceling without rescheduling:

If you canceled with at least 24 hours notice, you’ll be refunded the cost of your consultation minus the card processing fee (2.9% plus $0.30). Unfortunately, no refunds can be issued for appointments canceled with less than 24 hours notice. (If you’re canceling due to an emergency or unforeseen circumstance just let me know and I’ll be happy to make an exception for you.)
